The purpose of this job is to accomplish clients’ care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress as well as facilitating interdisciplinary approaches to deliver on the Fund’s mandate.
Job Title: Graduate Trainee – Case Management

Specification
•Attend to Road Transport Accidents victims in the various Clinics, Healthcare Centres and Hospitals within the designated region
•Injury profiling, through engaging Doctors for management of patient
•Evaluate patient to determine care, when the patient should be discharged, home planning, and other needs and action plans;
•Facilitate investigations on medical and treatment referrals;
•Help patients understand their conditions and treatment options;
•Interpret medical documents;
•Maintain appropriate cost and excellent records of clients treatment;
•Assess patient’s progress throughout hospital stay;
•Refer cases where patients and/or family would benefit from counselling;
•Facilitate provision of assistive devices;
•Conduct hospital visits;
•Submit daily, weekly, monthly and annual reports to Manager;
•Interpret and prioritise interventions on treatment plans
•Assist on lodgement of claims;
•Monitor treatment and progress, determine care, when the patient should be discharged, home and provide transport costs for
Claimant
•Transmit all bills and quotations to Bill Auditor
•Consolidate different treatment plans and communicate to Claimant on interventions required
•Prepare a comprehensive case discharge report after completion of treatment
Requirements
•Bachelor of Nursing Science
•Valid licence with Eswatini Nursing Council
•Healthcare Professional
•Valid Driver’s Licence
• Relevant working experience in nursing & general care
• Case Management, Process analysis
• MS Office, problem solving, relationship building skills, effective communication, presentation & reporting skills, planning,
organizing and co-ordination skills, team player, results orientated, complete finisher, cooperative, positive minded and highly
adaptable individual
